Don't buy it on fleabay and only use moeny order if paying to Canada. Also from what I have heard this Mikobu sucks it does not unlock all rom 102's. I heard that the nexus blue is better than Mikobu. One more thing who ever is selling you this does know squat it is just an unlocker not programmer you have to buy that separate.
It's true that Nexus is a very high-end unlocker, but it is about twice as expensive as the Mikobu. Mikobu can unlock most revs for ROM102 card, it just depends on the script that you use and how well you tune you potentiometer. I try to buy cards at rev103 or rev105, they unlock in a matter of minutes with my Mikobu.
As for a programmer, you should get that separately. Any ISO 7816 programmer will do; it doesn't have to be well-known brand name. I, personally, have some unknown-make $35 programmer and it works fine.
